For children and young adults with special education needs, transition from one setting to another can be a significant challenge for the student, their family and for those providing programs and support services. Successful transitions are the result of a planned collaborative effort between the family, school, and community agencies involved with the child.
Entry to school and leaving school to a post secondary destination are significant transitions for a student that require extensive planning. ฐวธ็ณินฯ is committed to supporting these transitions in accordance with Regulation 181/98 in the Education Act as well as Program Policy Memorandum (PPM) 140 and 156.

Individual Education Plan (IEP): Transition Plans
Regulation 181/98 in The Education Act and Program Policy Memorandum (PPM) 156 mandate that a transition plan be developed for all students who have an IEP, whether or not they have been identified as exceptional by an Identification, Placement, and Review Committee (IPRC) and including those identified as exceptional solely on the basis of giftedness. The transition plan is developed as part of the IEP. It is a detailed and coordinated plan designed to assist the student in making successful educational transitions. Transition plans must include the following elements:
- specific goals for the student’s transition
- the strategies to be used and the actions required, now and in the future, to achieve the stated goals
- the person or agency responsible for or involved in completing or providing assistance in the completion of each of the identified actions
- timelines for the implementation of each of the identified actions
Community Partners
Through our longstanding partnerships, YCDSB works directly with York Region community services to support student transitions. This includes, supporting Transition Meetings, co-hosting information sessions, implementing training, and promoting awareness with students, staff, and parents.
